Blue Shield of California Essential 3000

The Blue Shield Essential 3000 plan contains an annual deductible of $3,000. There is no out-of-pocket limit for this plan, and no lifetime maximum. Three office visits per calendar year are offered at a $40 copayment. After the deductible, Blue Shield covers 100% of the negotiated rate. For other medical services, members are responsible for 100% of the cost, until the deductible is met. After the deductible is satisfied, Blue Shield will cover all of the expenses.  Prescription drug coverage for the Essential 3000 plan is for generic medications only
