The Active Start 35 plan features a $0 annual deductible and a $7,500 out-of-pocket limit, with no lifetime maximum. Preventive services are covered by Blue Shield at no additional cost to the member; for other covered services, members are responsible for 40% of the negotiated rate. Unlimited office visits are provided at a low $35 copay. This plan provides coverage for both generic and brand name medications. There is a $10 copayment for generic drugs, and a $35 copayment for brand name drugs.
