CURRENT MEMBERS:
The Smartsense Full Rx 750 PPO plan by BCBS of GA has an annual deductible of $750, with an out-of-pocket limit of $3,750. For Physician Office Services, insured individuals are offered the first 3 visits per calendar year at a low co-payment of $30. This coverage also includes comprehensive prescription drug benefits—-providing for brand name drugs as well as generic ones.
