Due to the recent health care reform measures, benefits for the Smart Sense Generic Rx 7500 plan by Anthem CO have changed. If you purchased this plan before March 23, 2010, the benefits outlined below apply to you. The Smart Sense Generic Rx 7500 plan, by Anthem Blue Cross Blue Shield of Colorado, consists of a $7,500 annual deductible and a $10,000 out-of-pocket limit. The lifetime maximum for covered services is $7,000,000. For the first three office visits, the deductible is waived, and insured members make a $30 co-payment for each visit. After three visits, and when the deductible has been met, members pay 30% of the negotiated rate. Prescription drug coverage for this plan is for generic drugs only, and there is no separate deductible.
