CURRENT MEMBERS:The annual deductible for the Anthem CO Smartsense Full Rx 500 plan is $500, with an out-of-pocket maximum of $3,000. For visits to the doctor’s office, Anthem allows 3 visits with the deductible waived at a $30 co-payment.  After 3 visits and once the deductible has been satisfied, members are responsible for 30% of the negotiated rate.
