Current Members
The individual deductible for the Anthem CA CoreGuard Plus 5000 plan is $5,000, and the yearly copayment/coinsurance maximum is $3,500. The out-of-pocket limit is $8,500. After the deductible is met, coverage is provided for office visits, along with professional and diagnostic services, at 50% of the negotiated rate. Prescription drug coverage is based on a tier system, which features varying cost levels for both generic and brand name drugs.
