Current Members
The Smartsense Plus Standard Rx 3500 plan, by Anthem Blue Cross of California, features a $3,500 annual deductible, $7,000 out-of-pocket maximum, and unlimited lifetime benefits. The first three office visits are provided with the deductible waived, at a $30 copayment. After three visits, and once the deductible has been reached, members pay 30% of the negotiated rate. Preventive services are covered at no additional cost. For professional and diagnostic services, hospital stays, and emergency room care, members are responsible for a 30% coinsurance (after the deductible). This plan contains an upgrade prescription drug package, which includes both generic and brand name medications. Members make a $15 copay for generic drugs; for brand name medications, a separate $500 deductible applies. Once the Rx deductible has been met, members make a $40 copay for brand name drugs (those listed Anthem’s Drug Formulary). Scroll down for a link of a complete explanation of coverage.
