Current Members
The Smartsense Plus Upgrade Rx 2000 plan by Anthem CA contains a $2,000 annual deductible. The out-of-pocket limit is $5,500, and there is no lifetime maximum. The deductible is waived for the first three visits to a doctor, at a $30 copay. After three visits, members are responsible for 30% of the negotiated rate. Preventive services are covered 100% by Anthem. For other covered services, such as x-rays, lab work, hospital stays, and emergency care, members pay a 30% coinsurance, after the deductible has been met.
