Current Members
The Smartsense Plus Standard Rx 1000 plan features a $1,000 annual deductible and $4,500 out-of-pocket limit. Lifetime benefits are unlimited, and members are responsible for a 30% coinsurance for covered services. There is a $30 copay for the first three office visits, and the deductible is waived. After three visits, members pay 30% of the negotiated rate. Preventive services, including well-child care, immunizations, and important health screenings, are covered 100% by Anthem CA. This plan contains an upgrade drug package. For generic medications, members make a $15 copayment. For brand name drugs, a separate $500 deductible applies before Anthem covers a portion of the cost. Once the Rx deductible has been met, members are responsible for a $40 copay for brand name prescriptions (those listed on Anthem’s Drug Formulary).
