Current Members
The Smartsense Plus Standard Rx 2000 plan by Anthem CA contains a $2,000 annual deductible. The out-of-pocket limit is $5,500, and there is no lifetime maximum. Anthem waives the deductible for the first three visits to a doctor; members make a $30 copay for each visit. After three visits, members are responsible for a 30% coinsurance. Preventive services are covered by Anthem at no additional cost to the member. For other covered services, such as x-rays, lab work, hospital stays, and emergency care, members pay 30% of the negotiated fee rate, after the deductible has been met.
