The Individual PPO Share 7500 plan by Anthem CA features a $7,500 annual deductible, which also serves as the out-of-pocket limit. The lifetime benefit for covered services is unlimited . After the deductible has been satisfied, members make a $40 co-payment for doctor’s office visits. For all other services, including inpatient/outpatient hospital visits and maternity care, Anthem covers 100% of the negotiated rate. Prescription drug coverage for this plan includes both generic and brand name drugs (there is separate deductible for brand name medications).
